Understand the local CSA process and are provided information on the timelines for receiving and reviewing referrals for services.
Be notified in a timely manner before the child is assessed or offered services.
Be provided written information in the parent's native language or that interpretation is provided.
Be advised that their written consent must be provided before beginning the provision of any services that are part of the IFSP, upheld by the appropriate appeals process, or authorized by law.
Be advised that they may read, review and receive written information regarding the child's record upon request, unless otherwise ordered by the court; and understand the process for receiving the information they wish to review.
Understand the responsibilities of all parties to maintain confidentiality. Be provided information on how to access IFSP designated services from local human services professionals and are aware of their right to choose the provider for those services.
Be aware of the right to review, disagree with and/or appeal any part of the child's assessment or service plan.
Understand the locality's appeal process to the CPMT if the FAPT and family members disagree on a course of action.
A copy of the grievance policy is available upon request.
Participate during the entire process during which a FAPT discusses their child and family situation.
