Personal Property Forms
Important Temporary Change DUE TO COVID-19:
Notarization requirements for the Personal Property forms listed below are temporarily waived. Submittal of all other supporting documentation is still required.
City offices are partially open beginning Monday, May 18th. Please call 757-382-6455 to determine if your transaction can be completed in person. You may still submit completed forms via mail, email, fax, or dropbox at one of our four office locations.
Please be aware that our forms must be completed, printed and signed before remitting to the Commissioner of the Revenue's Office (with the exception of any forms that can be submitted online). A completed and signed form can be either scanned and sent as an email attachment, faxed or mailed to the Commissioner of the Revenue's Office. Online forms provide the option of submitting the form electronically to our office. If you have questions relating to any of our Personal Property forms, please contact the Commissioner of the Revenue's Office.
