Data Disclaimer Form

In consideration for the use of the map(s) or geographic data provided by the City of Chesapeake (“the City”), the recipient (“the Customer”) agrees to comply with the following conditions:

  1. Customer agrees to use the map(s) and data provided for graphic and informational purposes only. Customer agrees that the maps and data will not be sold, copied, or disseminated. Customer will not alter the maps or data. Any additions made to the graphics or text shall be clearly indicated as required in Condition 4 below.
  2. Customer agrees not to use the maps and data in lieu of required legal documents.
  3. Customer agrees that he will not hold the City liable for, or bring any claim against, the City, its officials, employees or agents, for any errors in the map or data. The Customer understands that all maps and data are subject to site specific verification and that the City does not warrant the accuracy of any such maps or data.
  4. In using the maps and data for the purposes permitted under this Disclaimer, the Customer agrees to cite the City as the source of such maps and data. Any additional graphics or text added to the maps or data must be clearly indicated as the work of the Customer and not the work of the City.

Not all maps and data requested under the Virginia Freedom of Information Act (FOIA) must be produced. For example, the City is not required to create documents under FOIA. The City Attorney’s Office will be consulted if there are any questions concerning a particular FOIA request. If maps or data are requested under the Freedom of Information Act, a formal request must be submitted on company letterhead. Hard copy (custom-designed maps) requests will use the Map Request Forms and be logged by the GIS Analyst and kept on record for future reference. This Disclaimer will be reproduced in a single sheet form, including a signature block for the Customer to sign and date, stating that all conditions are understood and agreed to.
