**CARE is a twenty-four hour computerized phone service that will call the homes of elderly and handicapped up to three times a day to check up on them. After the resident answers the call, the system will identify itself and prompt the resident to press 1 if everything is fine and 2 if an alert number needs to be notified. This service is offered free of charge to residents of Chesapeake.

**Meals on Wheels of Chesapeake provides nourishing meals to residents of Chesapeake who are unable to provide, obtain or prepare their own meal due to disability, age or illness. Meals on Wheels delivers two meals per day (one hot meal, one cold meal) Monday through Friday. The cost is $3.40 per day for the two meals.

**The File of Life is a medical history card that is distributed by TRIAD. The card is placed on the senior's refrigerator and will notify the Fire Department and Emergency Medical Technicians of medication, medical insurance and who to contact in case of emergency. This service is provided free of charge.

VICAP was established to educate, counsel, and assist senior citizens on medical benefit programs, long-term care insurance and problems with Medicare, Medicaid and medical bill paying. This program was established in cooperation with the Virginia Department on Aging and the State Corporation Commission Bureau of Insurance. Defrays taxes for eligible senior and disabled homeowners. The exemption is granted on an annual basis for those applicants that qualify.  Applicants must apply for the program each taxable year by May 15th.  To schedule an appointment, contact the Commissioner of Revenue’s office at 382-6698 or 382-6734.

Energy Assistance Program

The Energy Assistance Program assists low income households that pay a high portion of household income for home energy.

Fuel Assistance
Provides assistance for purchase of home heating fuel, furnace re-starts, late charges, installation charges and connection fees. Applications are accepted the 2nd Tuesday in October through 2nd Friday in November at the Chesapeake Department of Human Services.

Crisis Assistance
Provides assistance to prevent or alleviate a crisis situation when there are no resources for senior citizens. Applications are accepted from November 1, through March 15th at the Chesapeake Department of Human Services.

Following Types of Crisis Assistance:

  1. Once per lifetime payment of primary heat utility security deposit.
  2. Purchase of portable space heaters for temporary use.
  3. Payment for emergency shelter when there is no heat in the house.
  4. Purchase or repair of heating equipment.

Adult Protective Services is mandated by the Code of Virginia. These services must be provided, regardless of income, to persons 60 years and older and incapacitated adults over the age of 18, who are reported to be abused, neglected and/or exploited. The Chesapeake Department of Human Services-Division of Social Services receives reports, conducts investigations to determine the need for protective services and provides or arranges for services needed. Reports may be made Monday through Friday, 8:00 a.m. to 5:00 p.m. by calling 382-2000 and asking for Adult Services. You may also call the state hotline number, 1-888-83-ADULT, after regular work hours.

Adult Services are provided to persons with disabilities 18 years of age and older, to persons 60 years of age and older, and to their families when appropriate. Adult services include case management and other assistance to help adults remain in their own homes, i.e.home-based care, transportation, adult day care, and others. Adults who need placement outside of their homes may receive a pre-admission screening to determine the need for nursing facility placement or may receive an assessment to determine the need for placement in an adult care residence.

Companion Services may be provided to an adult who is unable to care for him/herself without assistance. Eligibility for companion services is based on companion assessment and income eligibility. Currently, the financial eligibility guideline for one person is a maximum of $1157 gross monthly income. Once the adult is approved for services, an agency approved companion provider can assist with light housekeeping, meal preparation, shopping, laundry, medication management, bathing and dressing. This program is not intended to provide total care. The number of approved hours per week is based on the individual's need and the agency's guidelines.

Adult Foster/Family Homes are private homes which provide room and board, supervision and special services to an adult who has a physical, mental and/or emotional problem(s). The adult must be incapable of independent living. The homes and providers are certified by the Chesapeake Department of Human Services-Division of Social Services prior to placement and monitored following placement of an agency customer. The provider may assist the customer with daily living skills such as bathing, dressing, grooming, medication management and other personal care needs as required.

The Adult foster/Family Homes accept no more than three adults. This family-oriented program allows for more individualized attention and enables the customer to remain in the community.

An Auxiliary Grant (AG) is a financial payment, which supplements income for persons who live in an adult care residence (ACR) or adult family home. Eligibility for the program is determined by the local social services agency where the individual resided prior to admission to the ACR or adult family home. A joint project sponsored by Senior Services of Southeastern Virginia and The Peninsula Agency on Aging serving all of Hampton Roads as an advocate for older adults and their families. The Ombudsman Program resolves complaints and concerns by investigating issues involving quality of care, quality of life and residents rights in adult care facilities, nursing facilities and home health services. Their goal is to investigate the problem and provide assistance along with education and information on long term care services.

Identification cards are made for senior citizens 55 years and older on the third Wednesday of each month from 9:00 a.m. to 12:00 noon at the Chesapeake Senior Center. Registration requires two forms of ID. The Chesapeake Sheriff's Office and Parks and Recreation Department offer this service free of charge.

ComfortCare Home Health Services, a part of the Chesapeake Health family, has been offering the same quality care at home that people have come to expect from our acute care setting since 1983. As an extension of the hospital, our licensed and bonded staff comprised of nurses, occupational and physical therapists, speech language pathologists, dieticians, home health aids and social workers provide medical attention for patients requiring care in the home setting.

Services are provided in the City of Chesapeake and other surrounding areas including Norfolk, Virginia Beach, Portsmouth, and Suffolk. Provides individual and family counseling, as well as substance abuse assessments, evaluation, urinalysis testing, training and education to Chesapeake citizens, who are experiencing problems related to the abuse of chemical substances. Services inclusive of medical, social, methadone detoxification, shelter placements and primary care programming are also available.

  • Intensive Outpatient Program

    Provides a specialized group service designed for clients, who require more than outpatient counseling to maintain a drug free lifestyle. The service is provided 4 days a week in three-hour blocks of time per occasion. The care program is for a 4 to 6 week period with ongoing outpatient counseling provided during and after the period. Respite Resources

Provides support services for families to ensure that families receive a break from care-giving responsibilities for their family member with mental retardation. Services are provided to persons of all ages.

Family Care

Provides living arrangements for adults with mental retardation.

Residential Services Program

Provides community based residential services for adults with mental retardation. These services are provided in normal community surroundings and foster acceptance, social integration, independence and participation in community opportunities.

Resource Management and Case Management

Case managers assist individuals with mental retardation and their families by advocating on their behalf, identifying needs, developing individual service plans, coordinating services and monitoring service delivery through assessment, linking activities, office, home and in-site visits and prescription team reviews.

Day/Vocational Programs

Offers day support services for adults with severe to profound mental retardation, many of who experience additional complex physical and sensory disabilities. Structured training is available three to five days a week and is provided in the areas of functional community skills and prevocational skills. Emphasis is placed on community based activities as a critical component of the program to increase the clients' independence. Transportation services are provided.

Provides a continuum of mental health services designed to meet the needs of children, adolescents and adults.

Residential Services

Provides a wide array of living options for persons experiencing a mental illness. The goal is to provide optimal living arrangements in the least restrictive setting possible. Current options include:

  1. Supported Living
    Provides training and assistance to adults, who seek to maintain their own residence in their house or apartment and need to improve home management.
  2. Sponsored Placement
    Provides a single family home in which the adult lives with the provider as long as mutually agreed. The provider receives specialized training and subsidy from the Chesapeake Community Services Board.
  3. Respite
    Offers a temporary residence for up to 21 days to provide relief from the adults' current living situation as a means of intervention.
  4. Supervised Living
    Provides time limited residence, averages two years, in a home or apartment setting with 24 hour staff availability. Training in all areas of daily living is provided.

Resource Management

Provide support services to adult citizens of Chesapeake experiencing mental illness and to their families. These services include discharge and pre-discharge planning, outreach and linkage to community resources, education and advocacy, crisis intervention and medication monitoring.

Crisis Intervention

Crisis Intervention/Pre-screening services are available 24 hours a day, 7 days a week, including telephone counseling, face to face assessment, consultation, pre-screening for commitment, voluntary hospitalization, social detox and referral sources. Call 548-7000 between 8:30 AM and 8:00 PM

Day/Vocational Programs

Coastal Clubhouse provides Chesapeake citizens, who have had a psychiatric disability to become independent. Services provided are: Psychosocial Day Program, Transitional Employment Program and a Social Recreation Program.

Support Groups

  1. MESA Family Workshops
    This is a series of classes for families and friends, which provides education and information about schizophrenia, Bipolar Disorders and the impact of mental illness on the family; stress, communication and problem solving guidelines. Contact Mental Health Services at 548-7011 for additional information.
  2. Chesapeake Alliance For The Mentally Ill
    This is a support group of families, who offer emotional support to each other by sharing experiences, knowledge and information. Meetings are held the first and third Tuesday of each month at Coastal clubhouse, 216 Great bridge Boulevard from 7:00 PM to 9:00 PM. For additional information call 548-7011.
  3. Chesapeake Assistance Trust for Persons With Mental Retardation and Psychiatric Disabilities
    Provides affordable ways families can plan future needs of client when they are no longer available to provide support and funds for their needs.

Lifeline is the oldest and largest personal emergency response service in the industry. Over the past 20 years, million of subscribers have relied on Lifeline to enable them to remain independent in their own homes. By activating a Personal Help Button, worn as a pendant or on the wrist, a person can automatically dial the Lifeline Response Center and receive appropriate assistance for the situation; an ambulance, fire department, police or simply a loved one to come by and check on you. Lifeline subscribers pay a low, one time sign-on fee, plus a monthly monitoring fee. The Elder Care Locator is a nationwide, directory assistance service designed to help older persons and caregivers locate local support resources for aging Americans. This is a public service of the Administration on Aging, U. S. Department of Health and Human Services and is administered by the National Association of Area Agencies on Aging and the National Association of State Units on Aging. Health Watch is the exclusive Personal Emergency Response System provided by Bon Secours Private Care. This system may provide the vital link that allows clients to live independently at home and feel safe and secure regardless of their age or physical condition. Help is just a push-of-the-button away.

Health Watch looks after people of any age, but may be especially helpful for:
  • Patients who are recuperating at home (after back surger, stroke, etc.) Anyone with a medical condition or chronic illness (heart condition, diabetes, etc.)
  • People who are physically challenged
  • The elderly who feel more secure being monitored

Health Watch consists of 3 components: 1) personal help button or transmitter (necklace, bracelet, belt -clip); 2) help console which plugs into telephone line; and 3) response center. When a subscriber needs help, he or she follows the ABC's" A) push the help button on personal transmitter or console; B) the help console signals the Health Watch Response Center; C) a trained operator attempts to talk to the subscriber through the help console. The operators know the subscriber's situation and will contact the designated responders. If the responders are not available to assist, or the subscriber is not able to reply, EMS is dispatched to the home.

Health Watch is available throughout Hampton Roads. Subscribers pay a one-time installation fee and monthly service charge. Limited grant money is available for indigent clients. Clients and loved ones, who use the service agree that the "peace-of-mind" and "sense-of-security" Health Watch offers is well worth the cost.

Bon Secours Home care is a Medicare/Medicaid certified, JCAHO accredited full service home care agency. We are celebrating our 20th year providing "Good Help To Those In Need…..A Home!" Our services include: 1) comprehensive adult and pediatric skilled nursing, with specialists in infusion therapy, maternal-child and psychiatric nursing: 2) physical therapy, speech-language pathology and occupational therapy for rehabilitative needs; and 3) medical social workers and home health aides, when covered by insurance.

Skilled nurse and home health aide visits are made on an intermittent basis. The patient must need a medically necessary skilled service, which is ordered by a physician. Medicare and some other insurers require that the patient be homebound.

In hospitals or nursing care facilities, the discharge planner consults with the physician and obtains an order for home health services. The discharge planner then contacts our nurse liaison to assist the patient in transitioning from hospital to home. Referrals may also be called in from physician offices. Home health services are provided in Chesapeake, Norfolk, Virginia Beach, Portsmouth, Suffolk, parts of Isle of Wight, and Smithfield.

Bon Secours Hospice has the distinction of being the fist hospice in Hampton Roads. We are committed to providing compassionate care to terminally ill patients and their families. When curative treatment is no longer an option, receiving compassion, sympathy and comfort measures are important. Our goal is to meet physical, emotional and spiritual needs so that whatever time remains is valuable to the patient and loved ones.

Patients are referred by their treating physician, who continues to manage care. The hospice team includes nurses, social workers, nursing assistants, pastoral care and volunteers. Hospice benefits are available through Medicare and most insurance plans. For more information, please call 398-2338.

Bon Secours Private Care is a Medicaid-certified agency. Private Care is a nursing service, that provides one-on-one care from nurse's aides with case supervision by a registered nurse. Services can be provided to patients in their home or hospital room. Our program is valuable to patients, who are recovering from an illness or injury as well as benefiting those who need help with daily living activities.

Our services include the following: nurse's aide to assist with bathing and dressing; preparation of meals based on dietary needs for patients unable to cook; light housekeeping while caring for patient, to ensure living area is clean; and assistance with grocery shopping and other errands as needed.

Most private care is contracted by and paid for by the client. Occasionally private insurance companies, local social services or Medicaid will cover private care.
