File of Life Program
Emergency personnel often arrive at the home of a senior citizen, only to find him unconscious or confused. The minutes that pass may cost the life of the senior. For that, reason the Chesapeake Sheriff's Office is endorsing the File of Life Program. This program will allow seniors and people with chronic conditions to alert the police, fire, and emergency personnel that their vital information is contained in the red File of Life packet to ensure that immediate and accurate treatment is taken. The File of Life is distributed free of charge by a grant from Sentara Health Foundation.

The File of Life packet is comprised of three parts:

A red sticker that states File of Life. This is to be placed on or near the front door to alert emergency personnel that you have the File of Life.

A red magnet packet with File of Life written on the top edge, which is to be attached to the front your refrigerator. This is the location where emergency personnel will go to when they enter your residence or facility. The packet will contain a file that will list the vital information to be filled out by the senior.

The file is contained in the magnet packet. All information must be filled out in pencil due the constant changes in medications and medical information. This information includes:

Who to contact in case of emergency
Medications that you take
Allergies you have
Medical conditions
Your blood type
Your physicians name
Your preferred hospital
Insurance records
It is also recommended to carry this information when you are being treated by your physician. Thereby, eliminating the task of constantly carrying your medicine or your medical records with you.
